Before making a referral, read the Medical Inclusion Tuition Policy. Tuition requests made to the local authority must meet the criteria set out in the policy.
Parental consent
Make sure you have a completed and signed parental consent form ready to upload:
We cannot accept your referral without a completed and signed parental consent form.
