The DofE uses this data to monitor that it is reaching diverse young people in the community

I understand that the data supplied on this form and in eDofE and information about DofE activities recorded in eDofE will be used by the DofE Charity, the Licensed Organisation and DofE centre to monitor and manage DofE participation and progress by young people and manage and support Leaders.

I understand that the DofE Charity will use personal data to communicate useful and relevant information to either help participants complete a DofE Award, Leaders or Licenced Operators to run DofE programmes more effectively or help the DofE Charity to improve the quality and breadth of its programmes.

I understand that the DofE will send emails that contain information about the Charity, DofE negotiated privileged discounts and invites to events and other activities. However, if you would like to receive these emails you will need to opt in. Once you have opted-in to this you can opt out at any time by visiting www.dofe.org/preferences, or clicking the unsubscribe link that can be found at the bottom of all non-programme related email.

This section must be read, completed and signed by you and your child, before your child can be enrolled with The DofE.

Without this permission your child cannot be enrolled with The DofE.

Enrolment cost covers the welcome pack, eDofE account and access to ongoing support from Redbridge/SixFive Education The DofE staff as well as information about opportunities to complete your sections.

Your child becomes a DofE participant when the Youth Service creates their account on eDofE which is DofE’s online system. Participants record their DofE programme and activities, and uploads evidence to prove which section is completed. For more information, go to www.dofe.org.

• Details provided on this form and my involvement in activities will be stored on the Management Information System used by the Youth Service (currently IYSS) and monitored by the Youth Service. We may share your information with the Council’s internal departments so they can carry out their statutory roles and support our service (e.g. social care, housing, Complaints Team, Business Intelligence Team, Corporate Finance Team, Legal Services Department, Information Management Team).
• I agree to my information being used to enrol me as a participant on the DofE Award. I understand that I will be managing my programme using the online eDofE system. I acknowledge that this system has a set of terms and conditions that I agree to. These terms and conditions are available at www.eDofE.org.
• Details provided on this form will be used for a database mailing list to send me information about Youth Service and LBR activities. I understand that the data will be held in line with the Data Protection Act 2018.
• I understand that my Youth Service membership may be terminated I do not behave in a socially acceptable manner.
• Membership fees and charges are used to subsidise the programme on offer for young people. In the event of any surplus, the Service reserves the right to roll over to the next financial year and spend on activities for young people.
• I am aware if they take part in an activity not delivered by the Youth Service, I recognised the responsibility lies with my parents / carer is for my safeguarding and safety.
• The cost of the enrolment is non-refundable.

I give permission for the London Borough of Redbridge, SixFive Education to create a DofE on eDofE account and agree to all of the above points.

• Membership fees and charges are used to subsidise the programme on offer for young people. In the event of any surplus, the Youth Service reserves the right to roll over the membership fees to the next financial year and spend these fees on activities for young people.
• As the participant’s parent / carer I am aware that if my child takes part in an activity not delivered by the Youth Service, I am solely responsible for safeguarding of my child’s safety and for checking that the provider is appropriately managed and insured.
• I understand there will be additional fees for the expedition section and Residential section (Gold award only)
• The cost of the enrolment is non-refundable
