This form should be used to apply for SEN Home to College Travel Assistance. This service is for students who have special educational needs, disabilities or medical needs. Supporting evidence must be supplied with the application.
All first time applicants must have a completed Education Health Care Plan/Assessment.
The application cannot be processed unless this department has a copy of the Education Health Care Plan/Assessment and it is the parent/carers responsibility to ensure a copy is available.
Travel Assistance is provided at the department’s discretion and if agreed, could be provided in any of the following ways:
- free travel with an Oyster photo-card provided transport for London
- provision of a TfL travel card
- training to travel independently (walking and using public transport)
- allocation of a Personal Transport Budget paying agreed cost based on mileage from home to place of education
- in exceptional circumstances, the provision of a space on a suitable vehicle
Additional important information:
It takes 4 weeks to process a completed application form. A completed application includes the Education Health Care Plan and any other supporting evidence. This department may assess applicants. If assistance is provided, please note that it is reviewed regularly and is subject to change.
